CHS accepts Medicaid patients with Audiology Orders for Evaluations and who have received Medical Clearance. Physicians must be IMPACT compliant. Audiology Orders for Evaluations can be signed by a licensed physician, a physician assistant, or a nurse practitioner. Medical Clearance Forms must be signed by a licensed physician.
